Parent Permission Form with Insurance Information

Proof of Residency - BGE, Water, or Cable Bill (from the last 3 months) or a copy of a current lease or mortgage

Trouble Uploading Forms....take pictures and email them along with the name of the athlete and the sport to the Athletic Director - mlerner@hcpss.org.

Academic Eligibility Requirement:  2.0 (minimum GPA) for 4th quarter for all returning high school students.  Rising (1st time) 9th graders are eligible for fall sports regardless of 4th quarter grades
